State of agency field forms (2026)

By Ben Hinton. Published .

We checked, in all 50 states, what format the state publishes three required field-recording forms in: the archaeological site form and the historic building form from the state historic preservation office (SHPO), and the construction stormwater inspection report. Crews fill these forms in on site, then retype or reformat them to match whatever the agency accepts, so the format decides how much of that work is copying.

Key findings

  1. 29 of the 93 state historic preservation office forms we could verify (31%) can only be submitted through an online system, with no blank form to download: 14 of 47 archaeological site forms and 15 of 46 historic building forms. The 7 SHPO forms we could not verify are left out of these counts.
  2. Of the 82 blank forms that agencies publish for download across all three categories, 36 are fillable PDFs (44%), 33 are Word documents (40%) and 13 are flat PDFs with no fields to type into (16%). 4 of the fillable PDFs are XFA forms that can only be filled in Adobe Reader.
  3. 16 of the 46 states we could verify (35%) publish no construction stormwater inspection form. Their permits list what an inspection report must contain and leave the form to the permittee. 4 states could not be verified.
  4. 12 SHPO forms in 8 states (Arkansas, Connecticut, Delaware, Illinois, Massachusetts, Mississippi, Rhode Island and Tennessee) have no blank copy published online. Where the office says how to get one, it is by request.
  5. Some forms in current use carry old revision dates: Connecticut's prehistoric archaeological site form reads "NEW 9/77", the Arizona State Museum archaeological site card reads "Rev. 12/3/93", Colorado's architectural inventory form reads "Rev. 9/98", Minnesota's archaeological site form reads "Rev.: 7/1/09" and Arkansas's construction stormwater inspection form reads "Revised date: 11/01/2011". Arkansas reissued its construction permit in 2021 and again effective November 1, 2026, without changing that date.
  6. Utah publishes its archaeological site form as a fillable PDF, but requires submitted site forms to be flattened PDF/A files with no fillable fields, uploaded through its e106 portal.
  7. In the 3 states where EPA issues the construction stormwater permit (Massachusetts, New Hampshire and New Mexico), the inspection form is EPA's Word template, which EPA describes as a template to customize rather than a finished form.

Percentages use only rows we could verify. 11 of the 150 rows are unverified, usually because the agency site blocked automated access or needed a login, and are counted separately below.

Archaeological site form. Alabama Historical Commission / Alabama State Site File (Office of Archaeological Research, University of Alabama). The Alabama State Site File is maintained by UA's Office of Archaeological Research (the AHC Section 106 page refers inquiries to sitefile@ua.edu). The OAR site-file page serves a CAPTCHA challenge to scripted requests and returns HTTP 405 to WebFetch, so Could not confirm whether a blank site form is downloadable. Neither AHC's site nor web searches turned up a public blank form. OAR also offers a subscription web version of the site file. Format left null because the site blocks access.

Historic building form. Alabama Historical Commission (SHPO). No revision date is printed on the form. The file name carries the date 2020-09-15, and the PDF was created 2020-09-15. The landing page also links separate instructions (Instructions_4_Completing_the_AHC_Building_Survey_Form3_14.pdf).

Stormwater inspection form. Alabama Department of Environmental Management (ADEM). Listed under 'Optional Forms' on the ADEM Water Forms page, so ADEM provides it but doesn't mandate it. It went with the April 1, 2026 reissued ALR100000 CGP. No revision date is printed. The PDF was created 2026-02-18. It replaces the older ADEM Form 023 (CSW-CGP-InspectionReport.pdf). The form says it must be completed by a QCI or QCP and certified by the permittee's responsible official.

Archaeological site form. Alaska DNR Office of History and Archaeology (SHPO), Alaska Heritage Resources Survey (AHRS). Adobe XFA dynamic form (LiveCycle Designer ES 9.0) with 34 fillable fields. It fills in Adobe Reader; most browser and mobile PDF viewers show only a 'Please wait...' placeholder page.

Historic building form. Alaska DNR Office of History and Archaeology (SHPO). Adobe XFA dynamic form (LiveCycle Designer ES 9.0) with 40 fillable fields. It fills in Adobe Reader; most browser and mobile PDF viewers show only a 'Please wait...' placeholder page.

Stormwater inspection form. Alaska Department of Environmental Conservation (DEC), APDES Stormwater. DEC's site sits behind DataDome bot protection: curl, WebFetch and headless Chromium all got HTTP 403 or a captcha, including direct file links. Search results show DEC publishes a 2026 CGP SWPPP template (https://dec.alaska.gov/media/n5lfs3j1/2026-cgp-swppp-template.docx) but turned up no standalone DEC inspection report form. Could not open the template to check for an inspection-form appendix. Alaska DOT&PF has its own SWPPP Construction Site Inspection Report (Form 25D-100), but that's a transportation-agency form and not used here. The 2026 AKR100000 CGP took effect 2026-02-01. Format left null because the site blocks access.

Archaeological site form. Arizona State Museum, Archaeological Records Office (state site file under the Arizona Antiquities Act; AZSITE partner with SHPO). The file is a legacy binary Word .doc. The classifier doesn't read pages or fields for .doc, so those are null. Its internal metadata claims 1 page, but the content has Sides A through C or more, so the metadata looks stale. The .doc was last saved 2004-11-29. The ASM 'Forms and Guidance' page lists it under Archaeological Records Office, next to a separate 'ASM Site Card Update' form (.docx, 1 July 2020) and an ASM Site Recording Manual. A 2019 ASM workshop deck says new sites can be documented on the site card or through the AZSITE Entry Module, and asks for one digital and one hard copy of the card. That's a mix of digital and paper, so submission is left as unknown.

Historic building form. Arizona State Historic Preservation Office (Arizona State Parks & Trails). The file is hosted on arizona-content.usedirect.com, the content store behind azstateparks.com, and is linked from the official SHPO forms page. That page links it twice (one link has a _1 suffix), and the two files are byte-identical. No revision is printed. The docx core properties show it was created 2002-10-01 and modified 2023-01-23. The form says: 'Send completed form to: State Historic Preservation Office, 1110 West Washington Street, Suite 100, Phoenix, AZ 85007'. The 21 fillable fields are Word content controls or form fields.

Stormwater inspection form. Arizona Department of Environmental Quality (ADEQ). static.azdeq.gov sits behind a Cloudflare JS challenge, so curl got HTTP 403. Downloaded the file with headless Chromium instead, and it's the same official URL. The form goes with the 2025 CGP (AZG2025-001, effective 2025-08-29), and its footer reads 'ADEQ 2025 CGP'. The landing page links it as 'CGP Inspection Report Form'.

Archaeological site form. Arkansas Archeological Survey (state site file, Registrar's Office). The page also offers a Word version (2025-ARAS-Site-Survey-Form.docx), plus a Site Survey Revisit Form (Jan 2024) and Site Form Instructions (Oct 2024). The forms page doesn't say how completed forms are submitted. A search-result snippet mentions mailing to the Registrar's Office in Fayetteville, but Could not confirm that on the page.

Historic building form. Arkansas Historic Preservation Program (SHPO), Division of Arkansas Heritage. AHPP's 2023 Survey Manual (https://www.arkansasheritage.com/docs/default-source/ahpp-documents/documentation-and-manuals/survey-manual-2023.pdf, 113 pp) names the Arkansas Architectural Resources Form as 'the principal survey form (Attachment A)', with code lists and samples on manual pp. 17-46. Found no standalone blank form on the AHPP publications or historic site survey pages, and web searches turned up none either. Contract language found in searches says AHPP provides the forms to survey contractors, so the form seems to be distributed on request rather than published. The manual says the first ten completed forms go to AHPP staff for review.

Stormwater inspection form. Arkansas Division of Environmental Quality (DEQ). The landing page calls it 'Inspection Form (Template)'. A Word version also exists at the same path (arr150000_inspection_form.docx) but isn't linked from the landing page. The form still carries its 2011 revision date even though the permit was reissued in 2021 and again effective 2026-11-01 (per the 2026 fact sheet, which now allows electronic preparation and retention of inspection reports). A search snippet says inspection forms are kept on site with the SWPPP and not sent to DEQ, but Did not confirm that wording on the page.

Notes

Archaeological site form. California Office of Historic Preservation. Recorded DPR 523A to match the study's example row. It's the base record every resource needs, and an archaeological site record is 523A plus the DPR 523C Archaeological Site Record (https://ohp.parks.ca.gov/pages/1054/files/DPR523C.docx, Word, footer 'DPR 523C (Rev. 1/1995)(Word 2/2015)'). Classifier caveat: its 'fillableFields' count for Word files counts every field code. Here the 3 are 2 PRIVATE codes and 1 SEQ code, not data-entry fields. For 523C the classifier reports 44, and all 44 are SYMBOL glyph codes. Both forms really have 0 fill-in fields. Records go to the regional CHRIS Information Center, but the forms page doesn't state a submission method. OHP offers the forms only as Word; no PDF is published.

Historic building form. California Office of Historic Preservation. Used together with the DPR 523A Primary Record. Word only. The docx core properties show it was modified 2013-11-19.

Stormwater inspection form. California State Water Resources Control Board. CGP Order 2022-0057-DWQ, Attachment D, section III.C.7 says: 'For each required inspection, dischargers shall develop and complete an inspection checklist that, at a minimum includes...' and lists the required content. Section IV.A says completed checklists are kept with the SWPPP on site or electronically, so they aren't submitted. No State Board inspection form is published on the construction stormwater or permit-reissuance pages. The fileUrl points to the permit attachment, not a form. The Lake Tahoe Basin has its own regional permit (R6T-2016-0010).

Archaeological site form. History Colorado, Office of Archaeology and Historic Preservation (OAHP/SHPO). OAHP keeps its forms in Google Drive, linked from its official resource-center Google Site (sites.google.com/state.co.us). The file was downloaded via drive.google.com/uc?export=download. A Word version is offered alongside it as a Google Docs link. The 1400 form is filled out for every archaeological resource, with component forms attached (1401 prehistoric, 1402 historical archaeology).

Historic building form. History Colorado, Office of Archaeology and Historic Preservation (OAHP/SHPO). Hosted on Google Drive and linked from the official OAHP resource-center Google Site. A Word version is also offered. Reconnaissance-level surveys use form 1417 instead.

Stormwater inspection form. Colorado Department of Public Health and Environment (CDPHE), Water Quality Control Division. Listed under 'Compliance assistance guidance documents' as 'Stormwater inspection report template' for COR400000. The file is served through CDPHE's Hyland OnBase DocPop viewer. curl only returns the HTML viewer, so the PDF was captured with headless Chromium. CDPHE's site also returns HTTP 403 to curl. No printed revision. The PDF was created 2025-03-03.

Notes

Archaeological site form. Connecticut State Historic Preservation Office (Dept. of Economic and Community Development). SHPO publishes two parallel site forms, 'For Historic Sites' (HIST-5 NEW 9/77, archaeology_historicsiteform.doc) and 'For Prehistoric Sites' (HIST-7 NEW 9/77). Neither is designated the main form; Recorded the prehistoric one. Both are legacy binary Word .doc files (saved 2010-01-10, metadata says 2 pages). They contain legacy Word FORMTEXT and FORMCHECKBOX fields, but the classifier doesn't count fields for .doc. The printed revision was read from the document's strings and still names the former Connecticut Historical Commission. A Designed Stone Landscapes form is also offered.

Historic building form. Connecticut State Historic Preservation Office (Dept. of Economic and Community Development). The SHPO's Survey Guidelines pages only link archaeology forms; the 'architecture' survey-guidelines page has no content. Found no building or structure inventory form on portal.ct.gov. Search results show copies of a 'Historic Resources Inventory - Building and Structures' form hosted by towns (e.g. westportct.gov), but those aren't official SHPO sources and weren't used. A SHPO State Register guide (portal.ct.gov/-/media/DECD/Historic-Preservation/Trails/how_to_list.pdf) is referenced in searches, but the link now returns a non-PDF.

Stormwater inspection form. Connecticut Department of Energy and Environmental Protection (DEEP). The 2026-01-01 CTR100000 general permit makes the permittee's Stormwater Pollution Control Plan include its own 'Plan Implementation Inspection Checklist' and 'routine inspection checklist' (Section 5.2.1.2.h). Reports are kept on site with the SPCP for 5 years and submitted on request. Solar projects (Appendix I), or others at the Commissioner's request, email reports to DEEP.StormwaterConstruction@ct.gov. The DEEP CGP page lists the permit, Appendices A-J, the fact sheet, and a dewatering monitoring and inspection guidance document (March 2026), but no inspection report form. The fileUrl points to the permit, not a form.

Archaeological site form. Delaware State Historic Preservation Office (Division of Historical and Cultural Affairs). Not published for download. The SHPO's surveys page says: 'Downloadable copies of the individual CRS forms are available as Word templates. Please contact the Historic Property Research Center Manager at (302) 736-7404 or jenifer.andersonreno@delaware.gov to obtain form templates.' So the forms are Word templates available on request only, and format is left null because the blank form couldn't be checked. The form number comes from the SHPO 'Data Coordination Guidance and Survey Forms Instructions' (Feb 2015), which lists CRS 4 as the Archaeological Site Form, used with the CRS-1 cover form. Per that guidance, final forms are attached to mapped points in the CHRIS online GIS, which is why submission is 'portal'. The guidance PDF and the documents.dnrec host both reject curl ('Request Rejected'), so Used headless Chromium.

Historic building form. Delaware State Historic Preservation Office (Division of Historical and Cultural Affairs). Same situation as CRS-4: the Word templates are only available on request from the Historic Property Research Center Manager, and no download is published. The Feb 2015 instructions list CRS 2 Main Building Form, CRS 3 Secondary Building Form and CRS 5 Structure Form, all used with the CRS-1 cover form. Completed forms are attached in CHRIS.

Stormwater inspection form. Delaware Department of Natural Resources and Environmental Control (DNREC), Sediment and Stormwater Program. An example form completed by the Certified Construction Reviewer (CCR). It says: 'This example form includes the minimum data and information reporting requirements. Other forms may be used as long as these minimum fields are included.' The file name still says 'Proposed'. The DNREC Feb 2025 listserv newsletter (used as the landing reference) announces the 'New Construction Site Review Report Form' on the Plan Review, Engineering, Construction and Maintenance Resources page. It also says that after 2025-02-28, reports must go through the online Asset Tracker database instead of email. Could not load that resources page directly (DNREC pages 404 or reject curl). The file was downloaded with headless Chromium. No printed revision.

Archaeological site form. Florida Division of Historical Resources, Florida Master Site File. Distributed as a .zip holding one file, Form_Archaeology_v50.pdf (zip entry dated 2025-02-19). That PDF is what was classified. It's a standard AcroForm, not XFA. A separate Guide to the Archaeological Site Form v5.0 (PDF) is posted, plus a Number Assignment Request form.

Historic building form. Florida Division of Historical Resources, Florida Master Site File. Distributed as a .zip holding one file, Form_Structure_v50.pdf, which is what was classified. The page also posts a separate Historical Bridge Form v5.0 and Historical Cemetery Form v5.0.

Stormwater inspection form. Florida Department of Environmental Protection (DEP), NPDES Stormwater Program. DEP doesn't publish a standalone inspection form. The inspection report form is the last page (p. 8) of DEP's 8-page 'Construction Stormwater Pollution Prevention Plan Template', linked as SWPPP 'guidance' on the CGP page. The pages count covers the whole template. The PDF dates from 2003 (created 2003-09-26), so it predates the current CGP. It says 'Inspections must occur at least once a week and within 24 hours of the end of a storm event that is 0.50 inches or greater'. Report contents are set by CGP 62-621.300(4)(a), Part 6.3, which requires signatures from the qualified inspector and a responsible authority.

Notes

Archaeological site form. Georgia Archaeological Site File (UGA Laboratory of Archaeology; state site file for GA HPD). Uncertain. The current Site Forms page doesn't link a file. It now says 'Please email gasf@uga.edu for a copy of the Official State Site Form', so the agency distributes the form by email on request. The 2020 PDF recorded here is still publicly reachable on the official UGA domain at the URL above (found via web search, not linked from the page).Submission is set to 'email' because the form is obtained through gasf@uga.edu, and the 2020 form's header lists gasf@uga.edu; the page doesn't spell out how completed forms are returned.

Historic building form. Georgia Historic Preservation Division (SHPO), Dept. of Community Affairs. Served as surveyform_hpd_nov2022.pdf (content-disposition). It's a paper field form; survey data is entered into GNAHRGIS, the online GIS database, which is why submission is 'portal'. Answer options are in a separate Survey Form Addendum.

Stormwater inspection form. Georgia Environmental Protection Division (EPD), Watershed Protection Branch. EPD's NPDES Construction Stormwater forms page lists the 2023 GAR100001/2/3 permits, fact sheets and 50-acre guidance, but no inspection report form. GAR100001 Part IV.D.4 sets the required inspection-report content (e.g. the report must identify BMPs that weren't properly installed or maintained, or certify compliance) and the signatory requirements in Part V.G. Reports are kept on site and provided on request. The fileUrl points to the permit, not a form. NOIs and other submittals go through GEOS.

Reconnaissance Level Survey - Survey Form
No form published
Notes

Archaeological site form. Hawaii State Historic Preservation Division (SHPD), DLNR. Hawaii has no standalone archaeological site inventory form. New sites get numbers through the SIHP request package: this 1-page Word .doc (legacy binary, so the classifier reports no pages or fields) plus a companion spreadsheet, '2021 SIHP Request Form - Archaeology, Architecture, & Burial Sites' (.xlsx). The form says: 'Submit this completed .doc with attached USGS map, TMK map... and site Plan(s)... fill out the .xls site data form... Email the request to your island's SHPD and attach forms to your HICRIS project.' So submission is email plus the HICRIS online system. Resource records themselves live in HICRIS.

Historic building form. Hawaii State Historic Preservation Division (SHPD), Architecture Branch. Uncertain whether this counts as published. The file is on the official SHPD domain (found via web search), but neither the current Forms page nor the Architecture Branch page links it; the branch page only links the 2018 Architecture Survey Guidelines. Those guidelines name three architecture survey forms: Historic Property Evaluation (HPE), Reconnaissance Level Survey (RLS) and Intensive Level Survey (ILS). SHPD tells the surveyor which one is required. A sibling ILS fillable PDF exists at .../2018/03/Intensive-Level-Survey-Form-2.2018_Fillable.pdf; the guessed HPE URL returned 404. The form's 2018 instructions say to email it to dlnr.intake.shpd@hawaii.gov. SHPD has since moved submittals to the HICRIS portal, so that instruction may be out of date.

Stormwater inspection form. Hawaii Department of Health, Clean Water Branch. The construction stormwater general permit is HAR 11-55 Appendix C; coverage is via NOI Form C, which is on the CWB Forms page. The CWB Forms page, HAR 11-55 page and permitting FAQ pages have no inspection report form. CWB's 'January 29, 2024 Construction Storm Water General Permit FAQs' (https://health.hawaii.gov/cwb/files/2023/05/January-29-2024-NPDES-Construction-Storm-Water-General-Permit-FAQs.pdf) says Appendix C Section 9.1.7.1 requires inspection reports within 48 hours and 9.1.7.2 requires signature by the Certifying Person or Authorized Representative, but it names no DOH form. Could not fetch the Appendix C rule text itself (Justia returned 403, and DOH links only the Appendix C fact sheet).

Notes

Archaeological site form. Idaho State Historic Preservation Office (Idaho State Historical Society). Idaho SHPO has replaced the paper ASI and IHSI site forms with direct entry in ICRIS (Idaho Cultural Resources Information System). The ICRIS page says ICRIS is used to 'enter survey and resource records', and field data can be captured in a Survey123 app (Pro accounts) and uploaded to ICRIS. Entering records needs an account and a signed confidentiality agreement. The SHPO 'Documents & Templates' list has no downloadable site form, only a Linear Resource Segment Form (.docx), a survey report template and code lists. There's an older transition document at https://history.idaho.gov/wp-content/uploads/ASI-form-to-ICRIS.pdf.

Historic building form. Idaho State Historic Preservation Office (Idaho State Historical Society). The IHSI building and structure form was folded into ICRIS in 2024, per search results describing the combined ASI and IHSI data. Buildings are recorded directly in ICRIS or through the Survey123 field app, and no downloadable building inventory form is posted. Login is required to enter records.

Stormwater inspection form. Idaho Department of Environmental Quality (DEQ), IPDES Program. Idaho DEQ is the permitting authority (IPDES CGP IDR100000, issued 2022-01-10). DEQ's CGP section lists 'Conduct regular inspections' and links the permit folder, which holds only the fact sheet, the permit and the LEW certification. For templates it links EPA's 'CGP SWPPP Templates' page (https://www.epa.gov/npdes/construction-general-permit-resources-tools-and-templates) instead of publishing its own inspection form. Inspection report content is set by IDR100000 Section 4.7. EPA is not the permitting authority here, so no EPA form is recorded.

Archaeological site form. Illinois State Historic Preservation Office (IDNR) / Illinois State Museum, Illinois Inventory of Archaeological Sites (IIAS). No public download. The SHPO 'Current guidelines for conducting archaeological research in Illinois' (https://dnrhistoric.illinois.gov/content/dam/soi/en/web/dnrhistoric/preserve/siteassets/pages/archaeology/archaeological-guidelines.pdf) say: 'It is imperative to submit new site forms and site revisit forms... See the IIAS viewer or contact DNR.Archaeology.Sitefiles@Illinois.gov for a copy of IIAS Submission Guidelines and submission forms.' So the form comes either through the credentialed IIAS online viewer (access needs proper credentials, via the SHPO Principal Archaeologist) or by email request. The guidelines also mention submitting GIS survey coverage to the ISM when getting site numbers.

Historic building form. Illinois State Historic Preservation Office (IDNR). Found no historic building or structure survey or inventory form on the SHPO site. The Survey, HARGIS and community-survey pages and the Forms & Applications page (which lists only permit and job forms) have none. HARGIS is the SHPO's online inventory database for viewing records. The SHPO does host a Preliminary National Register Evaluation Form (2026_preliminary-individual.pdf), but that's an eligibility request, not a survey or inventory form. The state CLG grant program says survey projects use an 'Inventory Form' coordinated with SHPO Survey staff, but no blank form is posted.

Stormwater inspection form. Illinois Environmental Protection Agency (IEPA), Division of Water Pollution Control. IEPA's Storm Water Forms page has no routine construction-site inspection report form for the ILR10 general permit. Its construction forms are the NOI, the NOT, and the 'Construction Site Storm Water Discharge Incidence of Non-Compliance (ION)' (WPC 624 Rev. 4/2025, fillable PDF, 2 pp, 26 fields; https://epa.illinois.gov/content/dam/soi/en/web/epa/topics/forms/water-permits/storm-water/documents/incidence-non-compliance-construction.pdf). SHAARD access restricted to qualified professionals. No downloadable site form on the DHPA archaeology page. The only downloadable archaeology form is the Archaeological Short Report (State Form 54566, https://forms.in.gov/Download.aspx?id=9540), which is a report form for records checks / Phase Ia, not a site inventory form.

Archaeological site form. Kansas Historical Society (Kansas SHPO). KSHS: 'Professional archeologists and CRM firms must record sites using the online site form only. Paper site forms will not be accepted from professional archeologists and CRM firms.' A paper form is still offered to amateurs/public at https://www.kansashistory.gov/research/pdfs/arch_site_form.pdf (classifier: flat_pdf, 3 pages, 0 fillable fields, text layer, pdfModified D:20110519111850-05'00', no printed revision date). Online form is hosted by the Kansas Geological Survey (ku.edu) for KSHS and linked from the KSHS page.

Historic building form. Kansas Historical Society (Kansas SHPO). KSHS 2026 HPF survey requirements: 'All survey forms will be submitted online through the Kansas Historic Resources Inventory (KHRI)'. Landing page: users register to submit new surveys; no downloadable survey form linked.

Stormwater inspection form. Kansas Department of Health and Environment (KDHE), Bureau of Water. KDHE's site returns HTTP 403 to WebFetch and curl, so the page was read from the Wayback Machine snapshot of 2026-04-28. That snapshot lists the 2022 CSGP permit, definitions, NOI and instructions, individual lot certification, NOT, transfer, erosivity waiver and fact sheets, but no inspection report form. The permit (S-MCST-2208-1 / KSR100000, effective Aug 1, 2022, also read via Wayback copy) lists minimum inspection report content (inspector name, date, BMP effectiveness observations, corrective actions) and requires reports kept on site and provided to KDHE or EPA on request. Live page could not be checked for changes after April 2026. KDOT Form 247 exists for KDOT projects but is not a KDHE form.

Archaeological site form. Kentucky Office of State Archaeology (University of Kentucky). OSA site record is a set of DOCX modules (01 Basic Information plus 02 Precontact, 03 Contact Period, 04 Postcontact, 05a/05b Cemetery, 06 Repeat Visit) with PDF instructions, all in a 2026-06 folder. Recorded the 01 Basic Information module as the main form. Footer revision text extracted as 'v2 06.01.2026'. Page says forms 'should be submitted according to OSA's current submission procedures' without naming a channel. The older Kentucky Heritage Council Archaeological Site Survey Short Form still appears on heritage.ky.gov.

Historic building form. Kentucky Heritage Council / State Historic Preservation Office. DOCX with legacy FORMTEXT fields. Separate Group Survey Form (2017) and Main Street/CLG form (2020) also published. Survey number requests go by email to khc-sitedata@ky.gov; submission method for completed survey forms not stated on landing page.

Stormwater inspection form. Kentucky Energy and Environment Cabinet, Division of Water. KYR10 (effective 1/3/2025, expires 12/31/2029) permit page links no inspection form; its SWPPP guidance page links the Kentucky EPSC Field Guide page, which lists 'Inspection Report' and 'Inspection Checklist' PDFs (Spanish versions also). The page does not say whether the form is for permittee self-inspections or agency inspectors; the form covers permit compliance items (NOI posted, SWPPP on site, BMPs). No printed revision date. Reasonable but not certain match to category.

Archaeological site form. Louisiana Division of Archaeology (Office of Cultural Development, CRT). Word template (.dotx) with content controls. Landing page says templates were revised October 4, 2021 and only the new templates are accepted since December 2021; no revision date printed in the form itself. DOCX core properties modified 2024-10-01. Also a compatibility version (Word 2010 and older) and a Supplemental Site Form. Draft forms 'are submitted to the Division'; channel not stated on the page (contact archsiteforms@crt.la.gov per search snippet).

Historic building form. Louisiana Division of Historic Preservation (Office of Cultural Development, CRT). No blank LHRI form is linked publicly. The landing page sends users to a Google Form ('one-stop Louisiana Historic Resource Inventory', https://forms.gle/2k9jF3wSedXjwgbMA) to request LHRI numbers and submit completed forms; it returned 401 (Google sign-in required), so the form file could not be retrieved. LHRI Guidelines & Instructions (June 2021, 16-page flat PDF) require a database spreadsheet, map, and 'PDF of the completed LHRI form(s)', and describe batch-exporting completed forms with Adobe Acrobat, which suggests a fillable PDF, but this was not verified. Non-Google submitters use an email address (obfuscated on page). submission set to portal (Google Form) per landing page.

Stormwater inspection form. Louisiana Department of Environmental Quality (LDEQ). No standalone inspection form. LDEQ's Blank Construction General Permit SWPPP template (legacy .doc, file saved 2017-02-14) includes a 'Construction Site Inspection Report' sheet in its Site Information section; the template states 'The inspection report will be reported using the Construction Site Inspection Report found in the Site Information section.' LDEQ says applicants are not required to use any specific SWPPP template. Template predates current LAR100000 (reissued effective Oct 1, 2024); permit Part IV lists nine minimum inspection report items, retained with SWPPP. Classifier returned format word but no page/field count for legacy .doc.

Archaeological site form. Maine Historic Preservation Commission. MHPC publishes two archaeological site forms: Prehistoric Site Form (recorded here) and Maine Historic Site Form (Historic Sites Inventory blank.pdf) for historic-period sites. No printed revision date found. maine.gov blocked WebFetch (403) but curl with a browser user agent worked. Forms page links a slightly different filename (..._0.pdf) for the same form; recorded the archaeology survey page link. Submission method not stated.

Historic building form. Maine Historic Preservation Commission. Flat PDF, created 2006-11-14 (no ModDate in metadata). MHPC also runs CARMA (Historic Property Workbench) online survey database; its linked user guide is dated May 1, 2008, and the forms page lists separate 'for Workbench' versions of some forms, so surveys may also be entered online. Submission channel for this form not stated on the forms page.

Stormwater inspection form. Maine Department of Environmental Protection, Bureau of Land Resources. MCGP page (permit effective Jan 14, 2025, expires Jan 13, 2030) links only the permit, appendices, and a webinar slide deck; no inspection form or log template. Appendix B(3) 'Documentation' requires a log (report) of inspections with inspector names/qualifications, dates, major observations, and corrective actions, made available to DEP on request. Archaeological site form. Maryland Historical Trust. Served as DOCX ('Maryland Inventory Archeology Form (4).docx'). Footers read 'Basic Data Form revised in: [DATE field]' showing 'April 26' and 'updated on: 14-Apr-26', but these are Word DATE fields that display the current date when refreshed, so they are not a reliable printed revision; printedRevision left null. Landing page: 'Send your completed documents to the Archaeological Registrar... The preferred method of submittal is as a Microsoft Word document'; registrar email is obfuscated on page, so submission recorded as email (electronic to registrar) with moderate confidence.

Historic building form. Maryland Historical Trust. Served as DOCX ('Maryland Inventory Architecture Form_0.docx'). No printed revision found. Landing page: 'Submit the completed electronic form and digital photographs to MHT via the appropriate Teams channel. You must mail a hard copy of the form with all photos to MHT.' So submission is both electronic (Microsoft Teams channel) and mail; recorded as mail because a mailed hard copy is required. Separate Addendum Form also published.

Stormwater inspection form. Maryland Department of the Environment. Linked on the current 20-CP page (permit effective April 1, 2023) as 'Construction Activity Inspection Form (.doc, pdf)', but the file sits in the 14GP folder and dates from Jan 2015 (written for predecessor permit 14-GP). Also available as .doc (https://mde.maryland.gov/programs/water/wwp/Documents/14GP/14GP-Inspection-Form.doc). Archaeological site form. Massachusetts Historical Commission. 950 CMR 70 defines the archaeological 'Inventory Form' as MHC Form D, and MHC guidance says archaeologists complete inventory forms for historic and prehistoric sites, but the MHC Forms page lists only Forms A, B, C, E, F, F-bridge, H, continuation sheet and NR criteria statement; no archaeological site form is published there. The live MHC pages are behind Incapsula bot protection (WebFetch and curl both got an empty challenge page), so the listing was confirmed from the Wayback Machine snapshot of 2026-02-23. MHC forms page: 'We only accept mail submissions at this time. We can't accept electronic submissions.' A third-party site (plymoutharch.com) hosts copies of the site forms; not official, not used.

Historic building form. Massachusetts Historical Commission. Legacy Word 97-2003 .doc (classifier gives format only for .doc; OLE metadata reports 2 pages, last saved 2012-12-04, template 'FORM B-complete.dot'). File downloaded live from sec.state.ma.us; forms page listing confirmed via Wayback snapshot 2026-02-23 because live HTML is bot-protected. MHC accepts mail submissions only, to 220 Morrissey Boulevard, Boston.

Stormwater inspection form. U.S. Environmental Protection Agency (Region 1). EPA is permitting authority in Massachusetts. Archaeological site form. Minnesota Office of the State Archaeologist (Department of Administration). Legacy Word 97-2003 .doc (classifier returns format only; OLE metadata reports 4 pages, saved 2015-09-08). Header still lists OSA at Fort Snelling History Center. Live mn.gov pages are behind a Radware bot captcha (WebFetch and curl blocked); landing-page content confirmed from Wayback snapshot 2026-06-12, and the file itself downloaded live. Page says forms 'must be filled out and submitted to the Office of the State Archaeologist' but the archived text did not name a channel. Site data also lives in the OSA Portal (qualified users).

Historic building form. Minnesota State Historic Preservation Office. SHPO 'About MnSHIP' page (Wayback snapshot 2026-10-10; live page behind Radware captcha): credentialed users can 'enter new historic inventory data directly into the system' and 'MnSHIP will also be the exclusive way to submit inventory information to the SHPO'. Older Individual Property Inventory Form described in the 2017 survey manual appears superseded. Wording 'will also be' leaves some ambiguity about whether exclusivity is fully in effect.

Stormwater inspection form. Minnesota Pollution Control Agency (MPCA). Optional checklist, stated to be 'an option for small construction sites'; large and linear projects need more extensive inspections. Body text still references the permit 'issued on August 1, 2018' although the current permit is the 2023 NPDES/SDS permit (wq-strm2-81a); footer date reads '9/9/26'. The guidelines (Rule 12.7.1) say site cards 'must be submitted to MDAH on appropriate card stock'; electronic versions may be submitted to get a trinomial, but a card is not final 'until a hard copy has been submitted' (recorded submission as mail; the page also says to email site cards to an obfuscated address). The guidelines' public download link (mdah.ms.gov/new/preserve/archaeology/archaeological-survey/) is a legacy URL not present on the current site.

Historic building form. Mississippi Department of Archives and History, Historic Preservation Division. No HRI form is published on MDAH's site. Per MDAH survey standards (16 Miss. Code R. 3-12.16, in the 2020 Mississippi Standards and Guidelines), HRI forms are produced via (1) the 'Consultant Inventory' Microsoft Access database provided by HPD, (2) hard copies or editable PDFs 'available from the HPD upon request', or (3) a pre-approved equivalent; completed forms are printed on 65# white cardstock for the permanent paper file (submission recorded as mail/hard copy).

Historic building form. History Nebraska, Nebraska State Historic Preservation Office (NeSHPO). Could not confirm. The SHPO Survey page (and the rest of history.nebraska.gov HTML) is behind a Cloudflare bot challenge that returned 403 to every automated fetch, so the page's file links could not be read. Archaeological site form. New York State Office of Parks, Recreation and Historic Preservation, Division for Historic Preservation (SHPO). New archaeological sites are recorded online in CRIS (Cultural Resource Information System): the Archaeological Survey form (5 steps), Step 4 'Resources' covers 'any archaeological resources that were identified or investigated'. Archaeology access needs CRIS archaeological-data privileges. SHPO's archaeology page (parks.ny.gov/shpo/archeology) offers no downloadable site form, only 'contact SHPO Archaeology staff'. Legacy NYS Historic/Prehistoric Archaeological Site Inventory Form PDFs are hosted only by a third party (nysarchaeology.org, NYAC/NYSAA) and were not used because they are not official. Classification is moderately confident: CRIS help does not use the words 'site form'.

Same printed instructions: submit a paper copy to SHSND and upload the PDF plus GIS shapefiles to the SHPO FTP site.

Stormwater inspection form. North Dakota Department of Environmental Quality, Division of Water Quality. Linked as 'Template' next to the NDR11-0000 construction permit (effective 2025-04-01). The template's printed date (07-2010) is older than the current permit, though the PDF was regenerated in April 2026 (now fillable). A Word (.doc) version also exists at the same path. Inspection records are kept on site; electronic records are acceptable per the permit.

Archaeological site form. Ohio History Connection, State Historic Preservation Office. OAI forms are completed and submitted only through Esri ArcGIS Survey123. The SHPO grants access to the OAI and OAI References forms when you email shpo@ohiohistory.org, per the SHPO's 'Survey123 - Getting Started' guide (https://www.ohiohistory.org/wp-content/uploads/2025/10/GettingStartedSurvey123_2025.pdf). The earlier I-Form platform was retired in 2023. Guidance: OAI v2 Guide (2025). No downloadable blank form. Landing page returned 403 to automated fetch; facts come from SHPO PDFs on the same domain.

Historic building form. Ohio History Connection, State Historic Preservation Office. OHI forms are completed and submitted in ArcGIS Survey123 ('Send Now'); access is granted when you email shpo@ohiohistory.org. The SHPO PDF 'Temp_OHI_forweb.pdf' (2023) is a one-page set of submission tips, not a form, and says printing instructions 'will be made available soon'. No downloadable blank OHI form found. Landing page returned 403 to automated fetch.

Stormwater inspection form. Ohio Environmental Protection Agency, Division of Surface Water. File is on Ohio EPA's asset server (epa.ohio.gov/Portals/35/storm). Header: 'Site Inspection Report / Ohio EPA Construction General Permit #OHC000006'. No printed revision; PDF generated May 2026. Could not load the landing page with automated fetch to confirm the exact link label (search snippets list a 'Construction Site Inspection Checklist (PDF)' under the construction section, which may be this file). Archaeological site form. Oklahoma Archeological Survey (University of Oklahoma), state archaeological site file. The OAS (the state's site-file repository, which assigns site numbers) is part of the University of Oklahoma, not the SHPO (Oklahoma Historical Society). Landing page links 'OAS Site Form (PDF), 2024'; there's also an Update Site Form and an Isolated Find Form (2018). No revision printed on page 1. Submission: send drafts by email to the Assistant State Archaeologist or as hard copy; send final forms as an unbound hard copy (mail) after the site number is issued. An older June 2019 version is still online but not linked.

Historic building form. Oklahoma State Historic Preservation Office (Oklahoma Historical Society). Form says 'Please type all data in uppercase - fields in red are required'. No printed revision date; PDF created 2015-12, modified 2026-03. Separate HPRI form for cemeteries; bridges use an ODOT drop-down Word form. Submission channel not stated on the forms page (the SHPO's Section 106 packet asks for the form plus photographs).

Stormwater inspection form. Oklahoma Department of Environmental Quality, Water Quality Division. Linked as 'Inspection Report' among DEQ's OKR10 Word templates (SWP3 template, corrective action report, grading/stabilization log, etc.). References OKR10 Part 4.5.13.E. Fillable fields counted are Word content controls/checkbox fields. No printed revision; DOCX metadata created/modified 2022-08-04. Word only, no PDF version. Archaeological site form. Oregon State Historic Preservation Office (Oregon Parks and Recreation Department, Oregon Heritage). The SHPO archaeology page links 'Oregon SHPO On-Line Site Form' (http://heritagedata.prd.state.or.us/#request.self) and an 'On-Line Site Form User Manual' (online_site_form_manual_dec2009.pdf). Site and isolate records are entered through the online system (account required); no downloadable blank site form is linked. OAR 736-051-0090 requires recording on a State of Oregon Archaeological Site Record submitted to SHPO.

Historic building form. Oregon State Historic Preservation Office (Oregon Parks and Recreation Department, Oregon Heritage). The SHPO Survey page says to submit information through www.makeoregonhistory.org; data is added to the online Oregon Historic Sites Database (heritagedata.prd.state.or.us/historic/). Professional reconnaissance/intensive surveys follow the 'Guidelines for Conducting Historic Resource Surveys in Oregon' and record properties in the Historic Sites Database. No downloadable survey form is linked from the current page. An older 'Historic Resource Record' Word form (oregon.gov/oprd/OH/Documents/HistoricResourceRecord.doc) appears in search results but is not linked from the current Survey page, so it was not used. Moderate confidence.

Stormwater inspection form. Oregon Department of Environmental Quality, Water Quality Division. The template says registrants may modify it; reports must be kept in chronological order in the site logbook. Built for the 2025-2030 1200-C permit (effective Dec 15, 2025). No dated revision string printed.

Notes

Archaeological site form. Pennsylvania State Historic Preservation Office (PHMC). New sites are recorded electronically in PA-SHARE (Basic, Pro or Business account); a tutorial covers 'Adding an Archaeological Resource to PA-SHARE'. The PASS page links no downloadable form. Per SHPO guidance found in search, PA-SHARE has been required since Feb 2021 and the old PASS PDF/Word forms (standard and collector) are no longer available. Older PHMC and third-party pages describing emailed PASS forms are outdated.

Historic building form. Pennsylvania State Historic Preservation Office (PHMC). The Historic Resource Survey Form (HRSF) was retired in February 2021; above-ground resources are now entered online in PA-SHARE (Inventory Form, Determination of Eligibility or Environmental Review wizards). 'Guidelines for Recording Buildings in Pennsylvania' (May 2022) includes a data-collection worksheet that SHPO says must NOT be submitted in place of the PA-SHARE online form.

Stormwater inspection form. Pennsylvania Department of Environmental Protection, Bureau of Clean Water. The native file in DEP's eLibrary is .docx (Word form fields); the eLibrary viewer also renders an on-the-fly PDF copy (flat, 4 pages), which was not counted as the source format. Covers PAG-01, PAG-02, Individual NPDES, Individual E&S and ESCGP permits. The older direct link on files.dep.state.pa.us now returns 404. DEP instructions allow permittees to convert the form for electronic use if content is unchanged, and permittees may keep using the version issued with their permit. Exact eLibrary folder/landing page not confirmed (found via search); DOCX metadata modified 2025-04-22.

Archaeological site form. Rhode Island Historical Preservation & Heritage Commission. RIHPHC's 'Guidelines for Archaeology in Rhode Island' (2021, https://preservation.ri.gov/sites/g/files/xkgbur406/files/2021-06/RI%20Arch.%20Standards%20and%20Guidelines%20-%202021.pdf) requires an RIHPHC site inventory form for every site and says it is 'available on request, and at www.preservation.ri.gov'. The current Archaeology Resources page links only the permit application (docx) and the guidelines; no downloadable site form was found anywhere on the site. preservation.ri.gov is behind a Cloudflare bot challenge (curl/WebFetch get 403); Read it with a headless browser. Form apparently available only on request from RIHPHC.

Historic building form. Rhode Island Historical Preservation & Heritage Commission. No historic building/structure survey or inventory form is posted on preservation.ri.gov. The site offers published town survey reports (PDFs) but no blank survey form. The old /historic-property-survey URL returns 'Page not found'. Site is behind a Cloudflare challenge (read via headless browser). Form may exist but is not published online.

Stormwater inspection form. Rhode Island Department of Environmental Management, Office of Water Resources. Legacy .doc (Word 97-2003); page count of 11 is from the file's OLE metadata, not the classifier. Its footer has a fill-in placeholder 'INSPECTION REPORT REVISION DATE MM/DD/YYYY, V.#', so no revision is printed. File metadata: created/saved 2015-03-31. The companion RIDEM Model SESC Plan fact sheet is dated Rev. May 1, 2015. The model SESC plan is voluntary; a site-specific plan and inspections (every 7 days and within 24 h of >0.25 in rain) are required under the RIPDES CGP. Reports are kept with the SESC plan; nothing found on routine submission. A separate inspection-report instructions doc (swpappfi.doc) and a small-sites template also exist. dem.ri.gov landing page is behind a Cloudflare challenge (read via headless browser); the file downloaded directly.

Statewide Survey of Historic Properties - Survey Form

Notes

Archaeological site form. South Carolina Institute of Archaeology and Anthropology (SCIAA), University of South Carolina - Office of the State Archaeologist. SCIAA keeps the state archaeological site files; the SC SHPO is at SC Dept. of Archives and History. The State Archaeologist page says to mail completed site forms to the Information Management Division, SCIAA, 1321 Pendleton St., Columbia. That instruction sits in the avocational database section, so whether it also covers professional forms is not fully explicit. A companion 2015 handbook and a separate simplified 'SC Avocational Site Reporting Form' are also posted. The underscore and hyphen URL variants serve the same file (md5 match).

Historic building form. South Carolina Department of Archives and History, State Historic Preservation Office. No revision date is printed on the form. The landing page describes it as 'SHPO Survey Form (new version, released November 2017)' and the filename says 2018. Surveys follow the Dec 2018 Survey Manual (errata 2019, 2022). The page also offers a blank Master Survey Database (MS Access) by email request. Surveyors deliver the form data via that database, but the page doesn't state how individual forms are submitted, so submission is 'unknown'.

Stormwater inspection form. South Carolina Department of Environmental Services (SCDES, formerly DHEC), Bureau of Water. The 2026 SC CGP (SCR100000, https://des.sc.gov/sites/des/files/Documents/BOW/Stormwater/SCDESCGP2026.pdf, Sec. 4.2.F) lists the minimum contents of weekly inspection reports (items I-XV), but no state inspection report form was found. The SCDES SWPPP Template (CSWSWPPPTemplate.pdf, flat PDF, 47 pp) includes only a one-line-per-inspection 'SWPPP Inspection Log', not a report form. Reports are retained in the on-site SWPPP, not routinely submitted (monthly summaries may be required case by case). Caveat: the SCDES 'Applications and Forms - Stormwater' page returned 'You are not authorized to access this page' (and 'Access denied' in a headless browser), so Could not confirm that no form is listed there. A Clemson Extension docx 'modified from an SCDES form' exists, but it is a compliance-inspection (regulator) report and not an official SCDES source.

Archaeological site form. South Dakota State Historical Society - Archaeological Research Center (ARC). ARC says the official form 'must be filled out and submitted' to get a new site number or record a revisit or update. It is offered as a fillable PDF or an MS Access utility (siteform-10.accdb), plus a variant with a continuation sheet. Only pages 1-2 are required. ARC accepts no forms based on other templates. ARC 'no longer accepts email submissions of any kind': materials go on DVD or flash drive with hard copies, so submission is recorded as mail. Guide: SiteFormManual.2024.V2.1.pdf.

Historic building form. South Dakota State Historical Society - State Historic Preservation Office. Architectural survey records are entered only through the SHPO's CRGRID Data online application (ArcGIS Experience; needs an SHPO-approved login). Legacy paper forms are retired. The page offers Word 'worksheets' (Structure/Bridge/Cemetery Form_2025.docx) that list fields and drop-down options for field use. The Structure worksheet classifies as word, 8 pages, 0 fillable fields. Recorded the format as online_portal because the worksheet is a field aid, not a form you submit; fileUrl points to the worksheet. The guidance is the 2025 Architectural Survey Manual. Intensive-level form templates are available by email request only.

Stormwater inspection form. South Dakota Department of Agriculture and Natural Resources (DANR), Surface Water Quality Program. The 2023 SD CGP (SDR100000) Sec. 4.6 requires an inspection report for each inspection and lists its contents. Its appendices are only NOI, NOT, Contractor Authorization, Transfer, Reauthorization and a precipitation map; none is an inspection form. The DANR construction stormwater page links no inspection form (NOI is via EPA CDX/NeT). Reports are kept with the SWPPP. SDDOT uses its own DOT-298 inspection form for DOT projects; that is not a DANR form.

The public can report sites by phone or email. Contact: TDOA.SiteFile@tn.gov. The form exists but is distributed only on request.

Historic building form. Tennessee Historical Commission (State Historic Preservation Office). THC moved survey data collection to an Esri Survey123 app that feeds its Historical Architectural Survey GIS System. Users download the app and the 'THC survey form' per the Survey123 Getting Started guide. No paper or PDF survey form is published. Guidance: The Tennessee Historical and Architectural Survey Manual (6/30/23), https://www.tn.gov/content/dam/tn/historicalcommission/106/thc_survey_manual_6.30.23_Final.pdf.

Stormwater inspection form. Tennessee Department of Environment and Conservation (TDEC), Division of Water Resources. TDEC has a named inspection form, CN-1173, required under the TN CGP (TNR100000; 2021 CGP expired 9/30/2026 per TDEC). Many copies are hosted by TN municipalities and universities (revisions seen: 2-13, 6-16, 12-21, 03/22). Could not find an official tn.gov download. On 2026-10-31 the TDEC Construction Stormwater Permit page links only myTDEC online forms (NOI, RFT/NOT, etc.) and shows an 'under construction' graphic; it has no CN-1173 link. Guessed tn.gov URLs returned 404. Search-engine snippets suggest earlier versions of the page listed CN-1173. Format left null because no official file was available; non-official copies were not used. Completed forms are kept on site with the SWPPP.

Archaeological site form. Texas Historical Commission (TexSite software) / Texas Archeological Research Laboratory, UT Austin (state site files). Texas has no paper or PDF site form. Sites are recorded in THC's TexSite, either the web app (xapps.thc.texas.gov/TexSite4, account login; 'Add Form' / 'Manage Forms') or a downloadable MS Access database (https://atlas.thc.texas.gov/About/TexSite). The data is exported to CSV/Excel and emailed with an ArcGIS shapefile to TARL (trinomials@austin.utexas.edu) to get a trinomial. Fee is $110 per site form (since Jan 2024). Recorded as online_portal because the web app is the primary entry tool; the Access DB alternative fits no format category.

Historic building form. Texas Historical Commission (State Historic Preservation Office). 'THC Survey Form (electronic version)' is a Word .docx form with 290 form fields/checkboxes. It has forms-only protection set, though enforcement is off. A separate 'printable version' is also listed, and the form is linked from thc.texas.gov/about/forms. No revision date is printed. Page count is from docx metadata. Guidance: THC Historic Resources Survey Manual. Submission method not stated.

Stormwater inspection form. Texas Commission on Environmental Quality (TCEQ). TCEQ has no standalone inspection report form. Archaeological site form. Utah State Historic Preservation Office (Utah Division of State History). The UASF (Parts A-D) replaced IMACS in 2018; manual dated 2018. Part A is the required core part; Part B prehistoric, Part C historic and Part D rock art are separate PDFs. The SHPO resources page links a SHPO Google Drive folder rather than its own server; files were downloaded from there. Per the UTSHPO Compliance Guidance (2024), the fillable PDFs were 'built and provided by UDOT'. Yet submitted site forms must be typed, flattened individual PDF/As that 'should not contain fillable PDF fields', submitted through the e106 online portal. No revision date is printed on Part A.

Historic building form. Utah State Historic Preservation Office (Utah Division of State History). Header reads 'HISTORIC SITE FORM (10-22)'. The same SHPO Google Drive folder also has a Word version ('Utah Historic Site Form 8-2022.doc') and a Section 106 short form with continuation sheets. Section 106 submissions to UTSHPO go through the e106 online portal per the 2024 compliance guidance. Non-106 survey submission is not specified.

'All Virginia site numbers are assigned through VCRIS'; new sites and revisits are entered in the VCRIS online system (help: https://vcris.dhr.virginia.gov/vcris/help/data-entry-archaeology.html). Access needs a licensed account (fees apply, with complimentary data-entry licenses available) and screening of archaeology users. No paper or PDF site form is published. DHR's 'PIF Archaeological Site' is a register-eligibility form, not a site recording form.

Historic building form. Virginia Department of Historic Resources (DHR). New buildings, structures and landscapes are recorded in the VCRIS online system, which also issues DHR ID numbers. No paper or PDF survey form is published. Architecture access is licensed (fees apply, with complimentary data-entry licenses). Section 106 review submissions go separately through ePIX.

Stormwater inspection form. Virginia Department of Environmental Quality (DEQ). It lists no inspection report form, and the 2024 VAR10 forms in 9VAC25-880 don't include one. Caveat: the single-family SWPPP template could not be downloaded (Akamai 403 even in a browser context), so Could not check whether it embeds an inspection form. Archaeological site form. Washington State Department of Archaeology and Historic Preservation (DAHP). WISAARD is 'the state's digital repository and data entry point for architectural and archaeological resources'; submitting inventory forms needs a WISAARD account. DAHP's 2018 'ARCHY FORM' tutorials cover entering the Archaeology Site Inventory Form in WISAARD. Trinomials are assigned once DAHP receives a completed form, which must be completed or approved by an SOI-qualified archaeologist. No downloadable blank site form is published (a 2018 tutorial notes it still references the old paper form).

Historic building form. Washington State Department of Archaeology and Historic Preservation (DAHP). Buildings and structures are 'submitted into Wisaard' as Historic Property Inventory forms. DAHP posts only reference aids (HPI Characteristics Table .xlsx, Architectural Description Guide), not a blank form. DAHP does not accept inventory forms for districts.

Linked as 'Archaeological Site Form' on the SHPO Archaeology Resources page, alongside a separate Isolated Find Site Form. Older Word/PDF copies remain on images.wvculture.org (siteform.doc/.pdf). The current page doesn't say how to submit; an older archived instruction sheet said to return forms to the SHPO in Charleston, but that was not verified as current.

Historic building form. West Virginia Department of Arts, Culture and History - State Historic Preservation Office. No revision is printed; '2026' comes from the filename and the PDF was created 2026-01-05. Consultants must request site numbers from SHPO staff before completing HPI forms. The submission method is not stated in the page or the instructions.

Stormwater inspection form. West Virginia Department of Environmental Protection (WVDEP), Division of Water and Waste Management. Archaeological site form. Wyoming State Historic Preservation Office / Wyoming Cultural Records Office. 'As of May 6, 2019, the Wyoming Cultural Properties forms should be submitted through WyoTrack on-line. The on-line versions of the forms are the WYSHPO submission standard.' Printable versions generated in WyoTrack go with Class II/III reports to the federal agency. 'Do not submit the old WYCPF in addition to the WyoTrack version.' WyoTrack accounts come from the Cultural Records Office (Laramie). No downloadable blank form is published.

Historic building form. Wyoming State Historic Preservation Office / Wyoming Cultural Records Office. Wyoming uses the same Cultural Properties Forms system for all cultural properties, all submitted only through WyoTrack since May 6, 2019. The SHPO page refers to 'the Wyoming Cultural Properties forms' generically and Did not log in, so the specific architectural/historic building component in WyoTrack was not verified. No separate downloadable building survey form was found.

Stormwater inspection form. Wyoming Department of Environmental Quality, Water Quality Division (WYPDES Storm Water Program). The CGP document list on the DEQ page loads via JavaScript from a Google Drive plugin; Read it with a headless browser. It has 12 items: LCGP and SCGP fact sheets/permits, NOI, NOTA, NPT, NOT, a 2020 SWPPP Template (.doc), erosivity waiver forms, SIC table and change-of-contact form. None is an inspection report form. The SWPPP template (.doc, 8 pp) only asks for an inspection schedule narrative meeting LCGP Part 10. Inspection requirements are set out in the permit text.

Methodology

What was checked. One form per category per state, chosen as the agency's main form: the archaeological site recording form from the SHPO or state site file, the SHPO historic building or structure inventory form, and the inspection report form under the state's construction stormwater general permit. Where EPA is the permitting authority, EPA's form was used. All 150 rows were checked on October 31, 2026.

How formats were assigned. Every downloaded file went through the same Python classifier (pypdf). A PDF with AcroForm or XFA fields counts as fillable. A PDF with a text layer but no fields is flat, and one with no text layer is scanned. In Word files, fill-in fields are content controls plus legacy FORMTEXT, FORMCHECKBOX and FORMDROPDOWN fields. “Online portal only” means the form can only be completed in a state web system. “No form published” means the agency lists what the report must contain but provides no form. “On request only” means no blank copy is published online. “Unverified” means we could not confirm the format, usually because the site blocked access or required a login. Unverified rows are not a format and are left out of every percentage.

Sources. The agency's own website and the files it links (some agencies host forms in Google Drive or a document-management system linked from their official pages), epa.gov, and the university-run state site files and university-hosted systems used in 10 states (Alabama, Arizona, Arkansas, Georgia, Iowa, Kansas, Kentucky, Oklahoma, South Carolina, Texas). Some state sites block automated downloads. Those were read with a headless browser or from Wayback Machine snapshots of the landing page, and the row's notes say so.

Limitations. Each state gets one form per category, the one the agency treats as its main form; many offices also publish supplements, component forms or a second format, which the notes mention. 11 rows are unverified. Portals and forms change, and a form we classified on October 31, 2026 may since have been replaced. Revision dates are the ones printed on the form, which can be older than the file itself.

Corrections. If a row is wrong or out of date, tell us with a link to the agency page and we will recheck it and update the data.

The format each US state publishes three required field-recording forms in (SHPO archaeological site form, SHPO historic building form, construction stormwater inspection report): fillable PDF, flat PDF, Word, online portal only, no form published, or on request only. 150 rows (50 states, 3 categories), each with the agency, form name, file and landing URLs, page and field counts, printed revision, submission method and source notes. 29 of 93 verified SHPO forms are portal-only; 16 of 46 verified states publish no stormwater inspection form.

Methodology
One form per category per state, chosen as the agency's main form: the archaeological site recording form from the state historic preservation office (SHPO) or state site file, the SHPO historic building or structure inventory form, and the construction stormwater inspection report form under the state's construction general permit (EPA's form where EPA is the permitting authority). All 150 rows were checked on 2026-10-31. Every downloaded file was put into a format by the same Python classifier (pypdf). A PDF with AcroForm or XFA fields counts as a fillable PDF; XFA forms are flagged in the xfa column because they only fill in Adobe Reader. A PDF with a text layer but no fields is a flat PDF; one with no text layer is a scanned PDF. Word fill-in fields are content controls plus legacy FORMTEXT, FORMCHECKBOX and FORMDROPDOWN fields. online_portal means the form can only be completed in a state web system. none_published means the agency lists the required report content but provides no form. none_found means no blank copy is published and the form is only available on request. A null format means the row could not be verified (the site blocked access or needed a login); null is not counted as a format. Sources are the agency's own website and files it links, epa.gov, and the university-run state site files and university-hosted systems some states use (Alabama, Arizona, Arkansas, Georgia, Iowa, Kansas, Kentucky, Oklahoma, South Carolina and Texas). Some state sites block automated downloads; those were read with a headless browser or from Wayback Machine snapshots of the landing page, and each such row's notes say so. Portals and forms change, and 11 rows remain unverified.
